Community Paramedicine
We take seriously the responsibility for keeping our community healthy. If you or a loved one requires more specialized medical services often associated with a trip to the hospital, our Community Paramedicine program brings some of those services directly to you in your home, avoiding duplicate services and unnecessary trips to the hospital.
We are available to answer questions, assess your needs and help you find community resources to help support your well-being. Community paramedics can also provide treatments and specialized diagnostic testing at your home.
Building relationships and providing advanced skills
In addition to paramedic education, NHRMC Community Paramedics complete an additional 300 hours of specialized training. Community paramedics are trained to identify and handle the special needs of their patients and are able to get to know their patients, helping to put them at ease, as well as to collaborate more effectively with all members of the patient's healthcare team.
How the Community Paramedicine program works
- Patients must be referred by their healthcare provider
- A Community Paramedic will call the patient following hospital discharge
- An appointment will be scheduled after the paramedic and patient have spoken
