Current Patients
If you are already a patient of this clinic, you can use MyChart or call during office hours to schedule an appointment.
New Patients
You need a referral from your primary care provider to be seen in this clinic. You can find one on our Providers Page.
Diagnostic studies performed in our clinic require an order from your primary care physician or a specialist involved in your care. If you need to schedule an appointment for a test that has been ordered, you can use MyChart or open scheduling through our physician finder, or you can call our clinic for assistance.
What to bring
We look forward to seeing you in our clinic. Please arrive at least 15 minutes before your first appointment. Bring with you:
- Your photo ID
- Your insurance card or the insurance card covering your child if your child is the patient
- A method of payment to cover your copay, or payment in full if your visit will not be covered by insurance
- Medications you are currently taking (or your child is currently taking)
- Recent test results that you have in your possession
