Appointments
To make an appointment with a provider in our office, call us during regular office hours, or scheduled online anytime using MyChart or open scheduling through our physician finder.
Please arrive at least 15 minutes early for your appointment.
New patients
While our clinic does not require that you get a referral to be seen, your insurance plan might require a referral in order for your visit to be covered. To learn more about coverage, please contact your insurer before scheduling your appointment.
What to bring
- Identification card
- Insurance card
- Copayment or payment in full if your visit will not be covered by an insurance plan
- Your current medications, including over-the-counter products
