Appointments
To make an appointment at our office, please call us during business hours. If you are already a patient of our clinic, you can use MyChart to schedule your visit using a smartphone or computer.
If you are a new patient, we ask that you come in 15 minutes before your appointment to fill out paperwork that helps us understand your medical history.
What to bring
- Your photo ID card
- Your insurance card, or the insurance card that covers your children
- A list of the medications you or your children take, including dosage information
- A form of copayment, or full payment if this visit will not be covered by insurance
