Outpatient services give you access to high-level care without a hospital stay
The care you need may be available without a hospital stay. Novant Health Mint Hill Medical Center’s outpatient care services can not only allow you to return to your home at the end of the day. These services also can potentially lower your healthcare costs. Ask your provider whether you might be eligible for outpatient services such as transitional care and infusion services.
Transitional care:
With a serious illness, you may need additional support and care once you leave the hospital. Transitional care may be available if you are well enough to be discharged but still have a high risk of returning to the hospital. Close follow-up care also may be appropriate if you were recently discharged and will have a wait time before you can get an appointment with your primary care provider. Transitional care can be appropriate if you are ready to be discharged but will need an infusion or other treatment the next day. The Transitional Care Clinic at Mint Hill Medical Center is a department of Novant Health Presbyterian Medical Center. Services are located on the first floor to the left of the main entrance.
Infusion services:
Infusion therapy allows you to get medications your doctor recommends without being admitted to a hospital for treatments. Outpatient infusion services can be a convenient option when you need to receive several treatments over time.
Infusions put medications directly into your veins, where they quickly enter your bloodstream. If you need intravenous (IV) therapy as part of your treatment plan, our team of skilled nurses, pharmacists and physicians will be ready to assist you in a setting designed for your comfort.
Infusions and injections are available for a variety of conditions, such as infections, chronic conditions such as Crohn's disease, rheumatoid arthritis, multiple sclerosis and asthma.
