Novant Health brings the best hospital care to you – in the comfort of your own home – with the Hospital at Home program. When you are healing from an illness or injury, we offer the same services, medications and treatments you would receive in one of our medical facilities with the comfort and convenience of being in the familiar surroundings of your home.

Conditions We Treat
Hospital at home services may be appropriate for patients with 75 eligible conditions including:
- Chronic obstructive pulmonary disease (COPD)
- Congestive heart failure (CHF)
- Cellulitis
- Sepsis
- Skin conditions
- Respiratory infections
- Concussions
Advanced Hospital Care at Home
Your Novant Health care team may recommend inpatient care at home if you still need advanced medical service but are stable and able to safely transition to receiving care at home. You will receive the same medical care that you would in a hospital setting with the benefits of being able to recover and heal in your own home.
Daily Virtual Visits
Your physician and care team will assess you daily through a virtual visit.
24/7 Monitoring and Continuous Support
Your care team monitors your vital signs, and you will have 24/7 access to a registered nurse who coordinates your care from a virtual hub inside the hospital.
In-Home Visits
Members of your Novant Health care team including community paramedics, physical therapists and other care staff will deliver services in your home depending on your individual plan.
About Hospital Care at Home
Only patients who meet criteria for hospital admission and have a qualifying condition are eligible for the Hospital at Home program.
In addition, in-home medical care services are best for patients who:
- Are 18 years of age or older
- Have a working phone number
- Can move around safely around their homes
- Have a trusted loved one in their home, close by to help
- Are comfortable using virtual options to engage with their care team remotely

- You will be sent home with the equipment you need to receive hospital care at home including a WiFi-enabled tablet and any medical equipment.
- Community paramedics will help teach you how to use the technology at home.
- Medications will be delivered to your home via courier.
- If you prefer, hospital meals can be delivered to your home.
- If you need hospital tests, the care team will coordinate your transport to and from New Hanover Regional Medical Center.
- The care team is available 24/7 to respond and coordinate support for any emergencies that might occur overnight or between visits.
- Once you are well enough for discharge from care, the care team will coordinate follow-up care with your primary care provider, review your after visit summary with you and collect equipment from your home.

Eligible patients who choose to receive in-home hospital care have reported higher levels of satisfaction and have experienced improved health outcomes. Being in a familiar setting can help with healing and faster recovery. Other benefits linked to hospital care at home include:
- Lower disruptions in sleep
- Reduced risk of hospital-acquired infections
- Reduced stress for patients and caregivers
- Fewer incidents of delirium – confusion some patients experience in the hospital
- Fewer incidents of falls
- Increased family involvement in care planning

The Hospital at Home program is covered for patients with Medicare, Medicaid and Humana Medicare Advantage.
Hospital at Home services are billed in the same way as an inpatient admission to the hospital. Depending on your insurance, you may be responsible for co-pays and other costs as determined by your health insurance plan.
