​​Novant Health brings the best hospital care to you – in the comfort of your own home – with the Hospital at Home program. When you are healing from an illness or injury, we offer the same services, medications and treatments you would receive in one of our medical facilities with the comfort and convenience of being in the familiar surroundings of your home.​

Conditions We Treat

Hospital at home services may be appropriate for patients with 75 eligible conditions including:

  • Chronic obstructive pulmonary disease (COPD)
  • Congestive heart failure (CHF)
  • Cellulitis
  • Sepsis
  • Skin conditions
  • Respiratory infections
  • Concussions

​​Advanced Hospital Care at Home

​Your Novant Health care team may recommend inpatient care at home if you still need advanced medical service but are stable and able to safely transition to receiving care at home. You will receive the same medical care that you would in a hospital setting with the benefits of being able to recover and heal in your own home.

24/7 Monitoring and Continuous Support

Your care team monitors your vital signs, and you will have 24/7 access to a registered nurse who coordinates your care from a virtual hub inside the hospital.

In-Home Visits

Members of your Novant Health care team including community paramedics, physical therapists and other care staff will deliver services in your home depending on your individual plan.
