Appointments
So that we can ensure your child’s first visit is as productive as possible, we ask that you receive a referral from a provider already involved in your child’s care. This allows us to match your child with the best provider for them. Once we receive your child’s referral information, we will contact you to schedule.
If your child is an established patient or you are seeking a second opinion, you can call us during clinic hours.
New Patients
We look forward to serving you and your family. Please arrive 15 minutes before your child’s first scheduled appointment so we can collect any information your child’s provider needs.
What to bring
- Your photo ID
- Your child’s insurance card
- Your method of copayment or payment in full if insurance will not cover your child’s visit
- A list of any medications your child is currently taking, including name and dosage
- Any forms (for a school or sports activity) that you need our office to complete
- Any forms you have completed ahead of time using the links below
