Appointments

You do not need a referral to be seen in our clinic. We see adults for all orthopedic injuries and conditions, as well as injured children. To schedule a visit, please call us during office hours.

If you have had a previous surgery, we will need to review your medical records prior to scheduling to ensure you get the care you need.

New Patients

We look forward to serving you. After you schedule your first visit, you will receive information about completing necessary new patient paperwork.

We ask that you arrive 15 minutes before your first visit so we can collect necessary information from you.

What to bring

• Your photo ID
• Your insurance card, or your child’s insurance card if we are seeing your child
• Your method of copayment or payment in full if insurance will not cover your visit
• Any previous imaging or recent medical records you have in your possession
• A list of any medications you are currently taking, including name and dosage
• Any forms that you need our office to complete
