Appointments

To schedule a visit, please call us during office hours.

While you do not need a referral to be seen in our clinic, your insurance might require one for your visit to be covered. Please confirm insurance coverage before scheduling.

New Patients

Once you call our clinic to schedule your first visit, you will receive information about completing new patient paperwork before you see us. Please arrive at least 15 minutes before your first appointment.

What to bring

• Your photo ID
• Your insurance card, or your child’s insurance card if we are seeing your child
• Your method of copayment or payment in full if insurance will not cover your visit
• A list of any medications you are currently taking, including name and dosage
• Any forms that you need our office to complete
