So we can ensure you get the care you need starting at your first visit, we ask that you receive a referral to our clinic from another provider involved in your care. Once we receive and review your referral information, we will contact you to schedule a visit.
If you are seeking a second opinion, please call us during office hours.
New Patients
We look forward to serving you. We ask that you arrive 15 minutes before you first appointment so we can collect necessary information from you.
What to bring
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any recent medical tests, scans or reports you have in your possession
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
