Appointments
We look forward to serving your family. If you have been referred to us by a provider already involved in your child’s care, we will contact you to schedule a visit as soon as we receive the referral information.
If your child is an established patient and needs follow-up care, or if you are seeking a second opinion, please call us during office hours.
New Patients
So that we can make your child’s first appointment as productive as possible, we ask that you arrive 15 minutes before your appointment time.
What to bring
- Your photo ID
- Your child’s insurance card
- Your method of copayment or payment in full if insurance will not cover your child’s visit
- A list of any medications your child is currently taking, including name and dosage
- Any imaging you have in your possession that we might not be able to access electronically
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
