Appointments

So that we can make your child’s first appointment as productive as possible, we ask that you get a referral from your child’s pediatrician or another provider already involved in his or her care. Once we receive a referral, we will contact you to schedule your child’s visit.

If your child is in need of follow-up care, please contact us during office hours. 

New patients

We look forward to caring for your child. Please arrive at least 15 minutes before your scheduled appointment so we can collect necessary information from you.

What to bring

• Your photo ID (such as a driver’s license)
• Your child’s insurance card
• Your method of copayment or payment in full if insurance will not cover your child’s visit
• List of current medications and supplements your child is taking, including name and dosage
