Appointments

To schedule an appointment with our clinic, please ask your child’s pediatrician for a referral to our office.  

Returning patients can use MyChart to schedule an appointment online or by calling us during office hours.

New Patients

Please arrive at least 15 minutes before your scheduled appointment. Arriving early gives you time to complete any necessary paperwork. 

What to bring

• Your child’s insurance card
• Photo ID (such as a driver’s license)
• Your method of copayment or payment in full if your visit will not be covered by insurance
• List of current medications and supplements your child is taking, including name and dosage
