Appointments
Once you receive a referral from your or your child’s primary care physician, our staff will contact you to schedule your introductory visit. To schedule a follow-up visit or to request an appointment to get a second opinion, call us during office hours. You can also schedule online using MyChart or open scheduling through our physician finder.
Your care team will make every effort to gather necessary information about your or your child’s health history before your visit. To make your visit as efficient and productive as possible, arrive at least 15 minutes before your appointment time. Bring with you:
- Your photo ID card
- Your insurance card or the insurance card covering your child if your child is the patient
- Your method of copayment or payment in full if your visit will not be covered by insurance
- Any medications you are or your child is currently taking
