Appointments

To make an appointment with our clinic, please call our office during clinic hours. Returning patients can make an appointment online using MyChart. 

A referral may be required by your insurance to schedule an appointment. Please check with your insurance company. If you need a referral to make an appointment, we will call to schedule once we receiver your referral information.

New Patients

Please arrive at least 15 minutes before your scheduled appointment. Arriving early gives you time to complete any necessary paperwork. 

What to Bring

• Photo ID (such as a driver’s license)
• Your insurance card
• Your method of copayment or payment in full if your visit will not be covered by insurance
• List of current medications and supplements you are taking, including name and dosage
• Any forms you need our office to complete
• Any forms you have completed ahead of time using the links below
