Appointments
If possible, please ask a provider already involved in your care for a referral to our clinic. This helps ensure your first visit is as productive as possible, and might be required by your insurance in order for your visit to be covered. We will call you to schedule once we receive your referral information.
You can also call us during office hours and we will be happy to help you schedule a visit. Returning patients can use MyChart.
New Patients
We look forward to serving you. We ask that you arrive 15 minutes prior to your first appointment so we can collect any necessary information.
What to bring
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
