Appointments
To see a provider in our clinic, you can schedule a visit through MyChart or call us during office hours.
While our clinic does not require a referral, your insurer might require one in order for your visit to be covered. Please confirm insurance coverage before scheduling your visit.
New Patients
We look forward to serving you! We ask that you arrive at least 15 minutes before your first scheduled appointment. This gives us time to collect any necessary information before you see your provider.
What to bring
- Your photo ID
- Your insurance card, or your child’s insurance card if we are seeing your child
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any recent orthopedic scans you have in your possession
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
