Appointments
We welcome walk-ins and make appointments upon request. To make an appointment, call our clinic during our normal business hours. We do not require a referral to be seen at our clinic.
Please arrive at least 15 minutes before your scheduled appointment. Arriving early gives you time to complete any necessary paperwork.
What to bring
- Photo ID (such as a driver’s license)
- Authorization form from your employer, if needed
- Method of payment for cash paid services
- List of current medications and supplements including over the counter and supplements
