Appointments
So that we can make your first appointment as productive as possible, we ask that you get a referral to our clinic from a provider already involved in your care. Once we have received your information and matched you with the right provider, we will contact you to schedule a visit.
If you need to schedule follow-up care or are seeking a second opinion, please call us during office hours.
New Patients
Please arrive 15 minutes before your first appointment so we can collect needed information from you.
What to bring
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
