Appointments
In order for your provider to have all necessary information before your visit, we require a referral from a provider already involved in your care. Once we receive your referral information, we will contact you to schedule your visit.
If you are an established patient and would like to schedule an appointment for follow-up care, please call us during office hours.
New Patients
We look forward to serving you in our clinic. We ask that you arrive 15 minutes before your first scheduled appointment so we can collect necessary information before your visit.
What to bring
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any CDs or files you have with previously completed diagnostic images (CT scans, MRIs, etc.)
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
