Appointments
If you are seeking diagnosis or care for an infectious disease for the first time, we ask that you get a referral to our clinic from another provider already involved in your care. This will allow us to ensure we match you with the best provider to meet your needs. We will contact you to schedule a visit once we receive referral information.
Established patients seeking follow-up care, or patients seeking second opinions, can call us during office hours to schedule a visit.
New Patients
We ask that you arrive at least 15 minutes before your first scheduled appointment.
What to bring
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
