Appointments

While our clinic does not require a referral to be seen, your insurer might require one in order for your visit to be covered. Please confirm insurance coverage prior to scheduling.

To schedule an appointment with a provider in our clinic, please call us during office hours.

We ask that you arrive 15 minutes prior to your first scheduled appointment so we can collect any needed information.

New Patients

We look forward to serving you. Before you can see a provider in our clinic, we will need to get any medical records related to your previous cardiology care. 

Once you schedule your first visit, you will receive instructions for completing new patient paperwork.

What to bring

• Your photo ID
• Your insurance card
• Your method of copayment or payment in full if insurance will not cover your visit
• Any previous cardiology-related medical records you have in your possession
• A list of any medications you are currently taking, including name and dosage
