We are committed to giving you easy access to cardiac care when you need it most. Schedule your appointment by calling our office during business hours.
Please call at least 24 hours in advance if you need to cancel or reschedule your appointment.
New Patients
We look forward to welcoming you on your first visit to Novant Health Heart & Vascular – Physicians Plaza (Electrophysiology). Please contact your primary care physician or cardiologist and request a referral, which is required by many insurers and our clinic policies. Once we receive your referral, we will call you to schedule your appointment.
What to bring
Please arrive at least 15 minutes early for your first visit. Our staff will check you in when you arrive. Please have these items ready when you check in:
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- Any medications you are currently taking in their original containers
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
