So that we can make sure you have the time you need with a provider, please call our clinic during office hours so we can understand your concerns and schedule your appointment accordingly.
New Patients
We look forward to serving you in our clinic. Please arrive at least 15 minutes before your first visit so we can collect necessary information from you. If you will be having a diagnostic test, make sure you do not apply lotions or oils before your visit.
What to bring
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- Any medications you are currently taking, including over the counter medicines, in their original containers
- Any forms you have completed ahead of time using the links below
