Appointments

To make an appointment in our clinic, please have a provider already involved in your care make a referral. Once we receive your referral information, we will contact you to schedule an appointment. 

If you are seeking a second opinion or need to schedule follow-up care, please call us during regular office hours. 

New Patients

We welcome new patients to our clinic. Please arrive 15 minutes before your first visit so we can collect any needed information from you.

What to bring

• Photo ID (such as a driver’s license)
• Your insurance card
• Your method of copayment or payment in full if insurance will not cover your visit
• A list of any medications you are currently taking, including name and dosage
