Appointments
We are a referral-based clinic, as we need certain information from a provider already involved in your care before scheduling your visit. Please have a provider who cares for you refer you to our clinic. We will contact you once we receive your referral information.
If you are seeking a second opinion, please call us during office hours.
New Patients
We ask that you arrive 15 minutes before your first scheduled appointment.
What to bring
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
