Appointments
So that your first visit is as productive as possible, we ask that you receive a referral to our clinic from a provider already involved in your care. Once we review your referral information and determine the best gastroenterology provider for you to see, we will call you to schedule an appointment.
If you are seeking follow-up care or a second opinion, please contact us using MyChart or call us during office hours.
New Patients
We look forward to serving you and ask that you arrive 15 minutes before your first scheduled appointment.
What to bring
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
