Appointments
To schedule an appointment with our clinic, please call us during clinic hours. If you have a MyChart account, you may also schedule an appointment with us online anytime through the portal.
New patients
We welcome new patients. Please arrive at least 15 minutes before your scheduled appointment so we can collect any necessary paperwork from you.
What to bring
- Photo ID (such as a driver’s license)
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- List of current medications and supplements you are taking, including name and dosage
