Appointments

We do not require a referral to be seen. We care for adults with orthopedic injuries and conditions, and children with orthopedic injuries. Please call us during office hours to schedule an appointment.

If you have had a previous surgery, we will review your medical records before scheduling to ensure you get the care you need.

New Patients

Once you schedule your first visit, you will receive information about completing new patient paperwork online.

We ask that you arrive at least 15 minutes before your first scheduled appointment so we can get any necessary information to your provider prior to your visit.

What to bring

• Your photo ID
• Your insurance card, or your child’s insurance card if we are seeing your child
• Your method of copayment or payment in full if insurance will not cover your visit
• Any previous imaging or recent medical records you have in your possession
• A list of any medications you are currently taking, including name and dosage
• Any forms that you need our office to complete
