Appointments

Our clinic does not require a referral. We see adults for all orthopedic injuries and conditions, and children with orthopedic or sports injuries. To schedule a visit, please call us during office hours.

We will need to review your medical records prior to scheduling if you have had previous surgeries to ensure you get the care you need.

New Patients

You will receive instructions about completing new patient paperwork online prior to your visit. 

Please arrive at least 15 minutes before your first visit so we can collect any needed information for your provider.

What to bring

• Your photo ID
• Your insurance card, or your child’s insurance card if we are seeing your child
• Your method of copayment or payment in full if insurance will not cover your visit
• Any previous imaging or recent medical records you have in your possession
• A list of any medications you are currently taking, including name and dosage
• Any forms that you need our office to complete
