Appointments
We are a referral-based clinic. Please ask a provider currently involved in your care to make a referral. Once we receive your referral information, we will contact you within 24 hours to schedule your first visit.
To schedule a follow-up visit, please use MyChart or call us during office hours.
If you are seeking a second opinion, please call our clinic during office hours.
New Patients
Please arrive at least 30 minutes before your first appointment so we can collect necessary information from you.
What to Bring
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
