Appointments
Appointments with our clinic can be made without a referral by calling us during office hours. You can also use MyChart anytime to schedule an appointment online.
If your insurance requires a referral, we will reach out to schedule an appointment once we have received your referral information.
New Patients
We welcome new patients to our clinic. Please arrive 15 minutes before your first visit so we can collect any needed information from you.
What to bring
- Photo ID (such as a driver’s license)
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
