Providing post-acute skilled nursing care where you need it

Being discharged from a hospital is a huge step in any patient’s recovery. But discharge doesn’t mean the need for care ends. Novant Health patients have access to skilled nursing care and therapy in skilled nursing facilities (also called SNFs) or in their own homes after leaving the hospital. Providers involved in your care or the care of your loved one may recommend care in a SNF or in a patient’s home depending both on recovery needs and each patient’s unique living situation.  

Novant Health works with a select group of SNFs and home health agencies that make up the post-acute network. Network participants work with Novant Health to improve patient outcomes after hospital discharge and to prevent hospital readmissions. Facilities in the network also agree to measure overall quality and performance, and maintain a high Centers for Medicare and Medicaid star rating.

Post-Acute Network

Novant Health collaborates with a select group of skilled nursing facilities (SNF) and home health agencies that we call our “post-acute network.” Participants in this network are committed to working with Novant Health to improve patient outcomes after discharge from the hospital, and to focus on what we can do together to prevent hospital readmissions. Facilities in this network also agree to measure overall quality and performance and maintain a high Centers for Medicare and Medicaid star rating.
