Appointments
So that we can collect necessary information before your first visit, we ask that you have another provider involved in your care refer you to our clinic. We will contact you to schedule your visit once we receive that referral.
To schedule follow-up care, please call us during office hours or use MyChart to schedule online. If you are seeking a second opinion, please call our clinic so we can help you.
What to bring
We look forward to serving you. Please arrive at least 15 minutes before your first visit. Bring:
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
