To make an athletic training or rehabilitation appointment, please call our office during business hours.
If you are coming to our office for rehabilitation services, we ask that you arrive at least 15 minutes early so you have enough time to finish important paperwork.
What to bring
For rehabilitation services:
- Your photo ID
- Your insurance card
- A list of the medications and supplements you currently take
- A form of copayment or full payment, if you are uninsured
For athletic training sessions:
- Debit card, credit card or cash payment for your athletic training session
