Appointments
We look forward to seeing you in our office. If you are new patient seeking care, please ask your primary care provider or another provider involved in your care for a referral to our clinic. Returning patients may use MyChart to schedule follow-up care, or call us so we can help you schedule.
What to bring
Please arrive at least 15 minutes before your first appointment. Bring with you:
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any recent results from scans or tests completed outside the Novant Health system
- Any forms you have completed ahead of time using the links below
