Well Appointments
Please call during business hours to schedule an appointment, or schedule online using MyChart. If you need to cancel or postpone your visit, please call at least 24 hours in advance.
Emergencies
For a life-threatening emergency, dial 911 immediately. For any other urgent needs, call our office during business hours. Should you need us after hours, a triage nurse will be available to assist you.
What to bring
Please bring the following to each appointment:
- Current medications, including any vitamins, herbal supplements or over-the-counter medications
- Your insurance card
New patients
Please bring a copy of your immunization and medical records and arrive at least 20 minutes early for your first visit. This will give you time to complete paperwork that is needed for the intake process.
