Please call our office to schedule an appointment. Please note that we require a 24-hour notice for cancellations.
If you are experiencing a life threatening emergency, dial 911.
For other urgent needs, call our office and speak to a member of our team. If your call is after normal office hours, call 704-210-5000 and ask for the physician on call for our practice.
All insurance co-payments and deductibles are due at the time of service. Our practice accepts most major insurance plans. If we do not have a contractual arrangement with your plan, you will be expected to pay in full at the time of service. Please contact our office to determine if we accept your insurance plan.
What to bring
Please bring the following to each appointment:
- A complete list of medications, including vitamins, herbal supplements or over-the-counter medications.
- Your insurance card and valid form of identification.
New Patients
We look forward to seeing you! If you are a new patient, please arrive 15 minutes early to complete paperwork and bring a copy of related medical records.
