Appointments
To see a provider in our clinic, please ask your Novant Health primary care provider for a referral. We will call you to schedule your first appointment once we receive that referral information.
If you are an established patient and would like to schedule a follow-up appointment, you can use MyChart or open scheduling through our provider page to schedule online, or you can call our clinic during office hours so we can assist you.
New patients
Please arrive at least 15 minutes before your first appointment.
What to bring
- Your photo ID
- Your insurance card, or the insurance card covering your child if your child is the patient
- Your method of copayment or payment in full if your visit will not be covered
- Any medications you are taking, or your child is taking if your child is the patient
- Any forms you have completed ahead of time using the links below
