Appointments

So that we can collect necessary information before your first appointment, we ask that you get a referral from your primary care provider or another provider involved in your care. Once we receive that referral, we will contact you to schedule your first visit.

If you have already seen a provider in our clinic and need to schedule a follow-up appointment, you can use MyChart or request an appointment on our provider page to schedule online. You can also call our office and we will schedule an appointment for you.

If you ever feel you might harm yourself or someone else, or you are concerned a loved one could harm himself or someone else, call 911 or head to your nearest emergency room. You can also contact our Outpatient Assessment Center at 800-718-3550.

New patients

We look forward to seeing you in our clinic. Please arrive at least 30 minutes before your first visit. Please be aware that your first appointment could be long, as we will complete a full assessment. If a child is the patient, a parent or guardian who can provide health and behavioral information will need to stay in our office the entire time. Bring with you:

What to bring

  • Your photo ID
  • Your insurance card or the insurance card covering your child if your child is the patient
  • Your method of copayment or payment in full if insurance will not cover your visit
  • Any medications you are currently taking, including over-the-counter medications
  • Any forms you have completed ahead of time using the links below
