To schedule an appointment with a provider in our clinic, use MyChart or open scheduling on our provider page. You can also call us during office hours so we can schedule an appointment for you.
If you are not able to keep your scheduled appointment, please call at least 24 hours in advance to cancel or reschedule. Failure to do so will result in a delay to schedule your initial visit. If you fail to cancel your appointment, you will be charged a missed appointment fee before your visit can be rescheduled.
Attending appointments is crucial to your care. Missing or canceling three appointments can result in dismissal from our clinic.

If you would rather complete forms ahead of time, you can use the links below to download them. If you have already completed the forms, arrive at least 15 minutes before your first visit. Bring with you to every visit:
What to bring
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- All medications you are currently taking in their original bottles
