Appointments
We look forward to seeing your child in our clinic! You can schedule an appointment online anytime using MyChart. You can also call us between 7:45 a.m. and 4:45 p.m. Monday to Friday.
What to bring
We welcome new patients! Please arrive at least 15 minutes before your first appointment to provide necessary information. You can also download and complete needed forms using the links below. Bring:
- Your photo ID
- The insurance card covering your child
- Your method of copayment or payment in full if insurance will not cover your child’s visit
- A list of any medications your child is currently taking, including name and dosage
- Any forms (for a school, sports or camp physical) that you need our office to complete
- Any forms you have completed ahead of time using the links below
