Appointments
Making an appointment at our clinic can be done online anytime using MyChart. You can also call us during office hours and our team will be happy to schedule a visit for you.
If your child would like a same-day appointment due to illness or injury, please call so that we can see you as quickly as possible.
What to bring
We look forward to welcoming new patients to our clinic. Please arrive at least 15 minutes before your child’s first visit. Bring:
- Your photo ID
- The insurance card covering your child
- Your method of copayment or payment in full if insurance will not cover your child’s visit
- A list of any medications your child is currently taking, including name and dosage
- Any forms (for a school, sports or camp physical) that you need our office to complete
- Any forms you have completed ahead of time using the links below
