To make an appointment with a provider at Novant Health Pediatric Sports Medicine – Matthews, call our office during office hours. Schedule online anytime using MyChart or open scheduling through our physician finder.
Schedule your first visit with or without a referral from your primary care provider. Your insurance plan may cover the cost of your visit. Our team will verify your coverage.
New patients
We look forward to seeing your child in our clinic. Please arrive at least 15 minutes before your first appointment. You can download and print your forms before your visit using the links below.
What to bring
- Your identification card
- The insurance card covering your child
- Your method of copayment or payment in full if your visit will not be covered
- Forms you need completed for school or sports participation
- Medications your child is currently taking, or any braces or orthopedic devices your child is currently using
