Appointments
You will need a referral to get an appointment for your child at Novant Health Pediatric Endocrinology – Midtown. Your child’s primary care provider or another provider involved in his or her care can provide a referral. This ensures we will have necessary information before your child’s first visit. We will contact you to schedule an appointment once we receive a referral.
To schedule a follow-up appointment or if you are seeking a second opinion, call our clinic during office hours. Schedule online anytime using MyChart or open scheduling through our Physician Finder.
We look forward to getting to know you and your child. Please arrive at least 15 minutes before your child’s first appointment so we can collect needed information from you. You can download and complete your forms ahead of time using the links below. Bring to every appointment:
- Your photo ID
- The insurance card covering your child
- Your method of copayment or payment in full if your child’s visit will not be covered by insurance
- Medications your child is currently taking, including over-the-counter products
- Recent test results that you have been given from outside the Novant Health system
