Appointments
To schedule a visit at our clinic, please ask your child’s pediatrician or your ob-gyn for a referral. If you are a returning patient, you can also make an appointment through a MyChart account or by calling our office.
What to bring
We ask that first time visitors to our clinic arrive at least 15 minutes early. This should give you time to complete needed forms before seeing your child’s provider. Please bring the following items with you to your visit:
- Your insurance card, or the insurance card that covers your child
- Your photo ID
- A way to pay for your child’s visit, if insurance will not cover the full amount
- A list of the medications and vitamins that your child takes regularly
