Appointments

To see a provider at Novant Health Orthopedics & Sports Medicine – Highland Creek, call our office during regular office hours. You can also schedule online using MyChart or open scheduling through our provider page.

We look forward to seeing you in our office. Arrive at least 15 minutes before your first appointment so we can collect information that is necessary for your visit. You can also complete some needed forms ahead of time using the links below. If you do not complete the forms ahead of time, we will provide those to you as we collect other information before your appointment.

Bring to every appointment:

• Your photo ID
• Your insurance card or the insurance card covering your child if your child is the patient
• Your method of copayment or payment in full if your visit will not be covered by insurance
• A list of any medications you are currently taking (or your child is currently taking)
• Any recent test results you have in your possession
