To schedule or reschedule an appointment, call our clinic during our normal business hours. You can cancel a visit by calling our clinic at any time and can leave a message after business hours or by selecting 3 during your automated reminder call.

If you are a MyChart user, you can take advantage of e-check in as well as cancelling appointments through your MyChart account.

What to bring

Please bring the following to your appointment:

  • Your insurance information/cards
  • A form of identification
  • A list of your current prescription and over-the-counter medications, and any herbal or nutritional supplements, including information about doses and frequency of use
  • If possible, please bring the bottles of any herbal or nutritional supplements you take regularly

New Patients

Welcome! We look forward to seeing you at your first appointment with one of our registered dietitians. If you will not be using MyChart’s e-check-in feature, please arrive 15 minutes ahead of your scheduled appointment to fill out all necessary paperwork.

Please note that a physician referral is preferred, but does not guarantee that your insurance provider will cover our services. You can speak to one of our patient services coordinators to get more information about what your insurance plan covers. If you are paying out-of-pocket, you will not need a referral.
