Appointments
If you would like to make an appointment, please ask your primary care provider to refer you to our clinic. Once your doctor has referred you, you can call our office during business hours to schedule a visit. If you would like to use your smartphone or computer to schedule, you can use MyChart or our online physician finder.
New patients
We ask that you arrive 15 minutes before your scheduled appointment time so you can complete necessary paperwork.
What to bring
- Your insurance card
- Photo identification card
- Forms you have completed before arriving
- A list of the medications you take, including vitamins and other supplements
- Your copayment, or your full payment if this visit is not covered by insurance
